Are you planning to travel abroad? If you are planning a holiday abroad and think you may need vaccinations or more general travel advice please complete a travel questionnaire which can be found under travel on the clinics and services page. Once we receive your completed questionnaire it will be passed to one of the travel nurses who will get in touch with you as soon as she can.


Are you Pregnant? If you have just found out you are pregnant, please click the link below to complete the maternity self-referral form. A midwife will then get in contact with you within 5 working days to make an appointment. https://www.uhmb.nhs.uk/maternity


 

New Patient Registration

All patients who live within the practice area (please see the map below) can register with the practice. Please contact the receptionist for further details.

Once you register with us your medical records are transfered from your previous practice, and as many are now sent electronically this means they should be available to the doctor at your first appointment. If you are taking regular medication it is  important to book an appointment with either a doctor or the nurse practitioner when you register to make sure that you do not run out of tablets. Please bring any medication you are on along to this appointment.
